  What is "The Plan"?

The Plan was formed on November 5, 2002, under an agreement between the United Food & Commercial Workers Union Local 480 and Signatory Employers to the Collectively Bargained Agreement. The Plan is used solely for the purpose of providing health and welfare benefits to employees, their families or dependents satisfying eligibility requirements as determined by the Trustees. It is subject to the provisions of the Employee Retirement Income Security Act of 1974 (ERISA), as amended. The Trust became operational on January 1, 2003.
